cost effectiveness of this type of dialysis, which was the main goal of our development of the technique in a cash-starved National Health Service in the United Kingdom. Dear Sir, Precedent claims are rarely correct, thus the statement by Drs Schlapper and DiazBuxo in the January 2005 issue of Blood Purifi cation [1] , that Uldall et al. [2] established the fi rst nightly home haemodialysis program in 1993, cannot go unchallenged. I believe that my group was the fi rst to develop this technique 30 years earlier and that we published extensively in the period 1963–1968 on this subject [3–7] . In addition, visual proof of my claim can be seen in a video of self unattended overnight home haemodialysis in 1968 by any of your readers at www.mybesthealth.com/shaldon/wmv/cahmed.wmv [8] . The first nocturnal home haemodialysis patient in Hong Kong.

We report our experience of the first use of nocturnal home haemodialysis in Hong Kong. The patient, a 40-year-old man with end-stage renal failure, was recruited into the Nocturnal Home Haemodialysis Programme at Princess Margaret Hospital in 2006. He received haemodialysis at home on alternate nights (3.5 sessions per week) for 5.5 to 6 hours per session. After 1 year of nocturnal home haemod...

Nocturnal haemodialysis in Australia and New Zealand.

Although early experience in Australia and New Zealand confirmed home haemodialysis to be well tolerated, effective and with lower morbidity and mortality compared with centre-based haemodialysis, the advent of ambulatory peritoneal dialysis and 'satellite' haemodialysis has led to a steadily declining home haemodialysis population. However, the emergence of nocturnal haemodialysis, as a safe a...

Home haemodialysis in Australia - is the wheel turning full circle?

In the mid 1970s, home haemodialysis accounted for nearly half of all patients on dialysis, both in Australia and elsewhere. The advent of both peritoneal dialysis (itself a home therapy) and satellite haemodialysis resulted in a gradual attrition in the use of home haemodialysis. Since 2000, the introduction of nocturnal home haemodialysis has begun to change this pattern in Australia, with a ...

Frequent prolonged home haemodialysis: three old concepts, one modern solution.

Most haemodialysis patients are, even when treated ‘adequately’ according to current standards, exposed to the so-called unphysiology of dialysis, which was described more than 25 years ago [1]. They are still at high risk of being overhydrated and dehydrated, hypertensive and hypotensive, acidotic and alkalotic, or hyperkaliaemic and hypokaliaemic thrice weekly. Furthermore, most patients are ...

Outcomes on home haemodialysis: registry challenges

Health care policy is encouraging expansion of home haemodialysis, aiming to improve patient outcomes and reduce cost. However, most patient outcome data derive from retrospective observational studies, with all their inherent weaknesses. Conventional thrice weekly home haemodialysis delivers a 22-51% reduction in mortality, but why should that be? Frequent and/or nocturnal haemodialysis reduce...
